It can be particularly helpful for NDIS participants, older Australians, families and carers managing ongoing or complex healthcare needs.

NDIS & Plan-Managed Participants

If you are plan-managed, you may be able to purchase a My Medical Folder through your NDIS plan, depending on your individual circumstances and plan requirements.

Step 1 — Confirm Eligibility

Please contact your Plan Manager or Care Coordinator to confirm whether a My Medical Folder can be purchased under your current plan.

Step 2 — Send Us Your Details

Once approved, please email us the following information:

  • Participant's full name
  • Delivery address
  • Phone number
  • Email address
  • NDIS number
  • Plan Manager's name and email address for invoicing

We will then provide the information required for your Plan Manager to process the purchase.

Step 3 — Payment & Dispatch

Once payment has been received from your Plan Manager, your order will be packed and dispatched within 1–3 business days.

Self-Managed NDIS Participants

If you are self-managed, you can purchase your My Medical Folder directly through our website.

Please retain your order confirmation and receipt for your records and, where applicable, submit these through your usual claiming process.

We recommend checking with your relevant NDIS contact if you are unsure whether the purchase is appropriate for your individual plan.
